Injectables

As we age the natural volume of youthful skin begins to diminish and wrinkles and folds start to form. Two of the most popular treatments offered in cosmetic dermatology are dermal fillers and BOTOX® injections. Cosmetic dermal fillers are injectable agents that are placed under the skin to restore the skin’s volume and smooth away facial wrinkles and folds. BOTOX® Cosmetic

Dynamic wrinkles are caused by contractions of delicate underlying facial muscles every time you smile, laugh or frown. BOTOX Cosmetic (abobotulinumtoxinA) is a botulinum toxin type A, a purified protein that is a byproduct of the toxin that will block the conduction of nerve impulses to muscles, causing a form of muscle relaxation in the area injected. BOTOX is safe and used for many medical treatments, such as hyperhidrosis (excessive sweating), migraine headaches, cervical dystonia, urinary incontinence, and many other therapeutic uses.

Lines that appear between your brows actually result from muscle movement and the passage of time. As skin becomes less elastic, repeated muscle contraction results in frown lines between the brows. BOTOX is a purified protein that smoothes the muscles by blocking nerve impulses. With a few tiny injections, BOTOX blocks the release of acetylcholine, the chemical that causes muscles to contract. With less movement, the skin surface gradually smooths out and the lines begin to fade. While technically not a filler, very tiny amounts of botulinum toxin are injected into specific facial muscles to immobilize or relax the underlying cause of lines and wrinkles associated with expression, particularly forehead creases, furrows between the eyebrows, and crow’s feet.

Dysport®

Dysport® (abobotulinumtoxinA), like BOTOX® Cosmetic, is an FDA-approved neuromodulator used for the temporary treatment of the wrinkles between the eyebrows. They are similar products but work in a slightly different way. Dysport® causes relaxation of facial muscles by blocking the communication between nerve endings and their surrounding muscles. It is injected into specific regions of the face to treat overactive muscles that cause dynamic wrinkles. The effects are temporary and repeat treatments will be necessary every 3-4 months to maintain the softened appearance of furrows and creases.

Xeomin®

Xeomin® (incobotulinumtoxinA) is FDA-approved for the treatment of wrinkles between the eyebrows. It can be used in a similar manner as BOTOX® Cosmetic. Xeomin® is closer to Botox® than Dysport® in terms of the manner in which it is used. As opposed to the other botulinum toxins on the market in the U.S., Xeomin® does not require refrigeration. Additionally, it is sometimes called a “naked botulinum toxin,” meaning that there is no additional protein content which may lower the risk of patients developing antibodies to the product. The effects are temporary and repeat treatments will be necessary every 3-4 months to maintain the softening appearance of furrows and creases.

Hyaluronic Acid Gels

Fashioned from a basic element in connective skin tissue, hyaluronic acid gel, along with collagen, is the framework in which skin cells live. Hyaluronic acid is a natural constituent of the dermis, the second layer of skin that lends structure, firmness, and volume to the skin. Although abundant in youth, with age and exposure to environmental factors such as UV light, the presence of hyaluronic acid in the skin decreases. Hyaluronic acid attracts water to the region where it is placed, thereby plumping the treated areas and decreasing the presence of facial lines, wrinkles, and folds.

Hyaluronic acid gel fillers work well to reshape and smooth the surface of aging skin, particularly nasolabial folds, smile lines, and drooping corners of the mouth. These temporary fillers restore fullness to the skin and reduce the appearance of moderate to severe facial wrinkles and folds, creating a smoother, more youthful appearance. The results appear to last six months to one year and longer.

Most hyaluronic acid fillers are now available with local anesthetic, which helps to increase comfort during injection. Ice packs, local numbing cream, and nerve blocks may be used if necessary to increase the ease and comfort of the procedure. An added benefit of hyaluronic acid fillers is that they can be reversed in the rare case of overcorrection with the injection of an enzyme called Hyaluronidase that breaks down hyaluronic acid fillers rapidly following treatment.

There is very little potential for reaction to hyaluronic acid fillers. Mild swelling and bruising are the most common side effects following injection and they resolve quickly. These side effects can be greatly minimized with proper injection technique.

Juvederm® XC

Juvéderm® hyaluronic acid gel filler is specially formulated with a smooth consistency to flow easily into the skin, creating a smooth, natural-looking correction. Hyaluronic acids such as Juvedérm® have now become the gold standard of fillers currently available. A long list of facial regions may benefit from injection with Juvedérm®. The nasolabial folds (nose to mouth lines), marionette lines (corners of the mouth), perioral area (around the mouth), upper cheeks, lips, hollows under the eyes, acne scars, and temples can all be treated with Juvedérm®.

Juvedérm® gradually degrades, and as it is broken down in the skin, the hyaluronic acid particles attract more water, allowing consistent wrinkle correction over the duration of the filler treatment. Results can last for 6-12 months or longer in most cases.

Juvederm® Voluma

Voluma is the newest hyaluronic acid filler of the Juvederm family of hyaluronic acid gel products. Juvederm Voluma provides volume to areas of the face that have deflated and become sunken over time, such as the cheeks and upper face. It lifts tissue, giving a lifted appearance to the face. Voluma has a different consistency from other hyaluronic acid products. It is composed of shorter chains of hyaluronic acid with connections called crosslinks. In general, the more crosslinks, the greater the lifting power of a hyaluronic acid filler. Voluma has demonstrated superior lifting capacity for sagging tissues compared to many other injectable fillers available. The product is also soft and natural-looking when injected, making it an excellent choice for facial shaping and contouring because it can be molded into place.

Due to the nature of its particle size and viscosity, Juvederm Voluma lasts longer than many other hyaluronic acid fillers. Results can last for one year or longer in most cases.

Restylane®-L

Restylane®, Restylane® Silk, and Restylane® Lyft (formerly called Perlane®) are composed of NASHA, a non-animal derived hyaluronic acid in a crystal clear injectable gel that attracts water to add supple volume to the skin. Restylane® is especially effective for restoring fullness to and softening lines around the lips, and improving nasolabial folds and marionette lines. Restylane® is available pre-mixed with the anesthetic lidocaine to increase comfort during an injection session.

Many areas of the face respond well to Restylane® treatment. This popular hyaluronic acid filler are used for the correction of moderate to severe facial wrinkles and folds, including the nasolabial folds, marionette lines, vertical lip lines, and corners of the mouth. Restylane® can be also be used to rejuvenate the eye area. Tear troughs result from a loss of soft tissue support in the under eye area, creating a dividing line between the eyelid and cheek zones to the face. Restylane® can add volume back to the tear trough area, creating a smoother contour from the eyelash line to the cheek. Hollows around the eyes, the temple region and the brow bone can also benefit from small amounts of Restylane.

Restylane® and Restylane® Lyft are slightly firmer hyaluronic acid gel fillers than Juvederm. The Restylane® family of products are classified as temporary fillers and typically last approximately 6 to 12 months and longer in some cases. As Restylane® or Restylane® Lyft filler break down, the remaining filler attracts water to the area of wrinkle correction, creating persistent wrinkle and facial fold correction. Multiple treatments will be needed to maintain the effects of Restylane injections.

Restylane® Silk

Restylane Silk is the newest hyaluronic acid filler to the Restylane family of products. Hyaluronic acid is a common component to the second layer of the skin, and it helps provide structure to the skin. Restylane Silk, as it name infers, is a sooth, silky formulation of smaller particle hyaluronic acid gel. It provides improvement of fine lines around the mouth and it can also be used to correct volume loss of the lips as well as smoker lines or fine vertical lines around the lips.

Restylane Silk differs from other products in the Restylane family by its particle size. It is considered a fine line filler. For example, Restylane Lyft (Perlane) has larger particles that makes it ideal for lifting tissues and creating deep volume in the face. Restylane has smaller sized particles that make it an extremely versatile filler. It can be used to enhance lip size and shape and for filling in folds and creases to restore facial volume. Restylane Silk has the smallest particle size, allowing it to smooth fine wrinkles such as the lipstick lines and for lip augmentation.

Restylane® Lyft (Perlane)

The Restylane® family of products are classified as temporary fillers and typically last approximately 6 to 12 months. As Restylane® or Restylane® Lyft filler dissipates, the remaining filler attracts water to the area of wrinkle correction, creating persistent wrinkle and facial fold correction. Many areas of the face respond well to treatment with Restylane® Lyft, including moderate to severe facial wrinkles and folds including the nasolabial folds, marionette lines, vertical lip lines, and corners of the mouth. Restylane Lyft is more viscous than Restylane so it is ideally suited for treating deeper folds, and enhancing facial volume around the cheeks, chin and jawline. It can last up to one year in most cases.

Belotero®

Belotero is a temporary hyaluronic acid filler in the same class of fillers as Juvederm, Restylane and Restylane Lyft. What makes Belotero unique is its area of placement in the skin. Unlike the other fillers in this category, Belotero is placed very high up in the dermis, the second layer of the skin. This makes this hyaluronic acid gel filler a good option for effectively improving fine lines.

Belotero is an excellent choice of filler for fine etched lines and wrinkles of the face, under eyes and around the mouth. It is excellent for improving vertical lip lines, marionette lines, nasolabial folds from the nose to the mouth, and chin lines. It is also helpful to reduce fine lines around the eyes and crow’s feet, some horizontal neck lines and creases between the eyebrows.

Belotero is made of a double-crosslinked technology that produces a smooth cohesive gel that degrades slowly over time. Results may last 6 months or longer.

Collagen Stimulators

This category of fillers is designed to extend the duration of traditional dermal fillers by stimulating new collagen formation that is lost with age.

Sculptra® Aesthetic

This effective volumizer is used to help restore youth by restoring the volume of the fatty tissue and bony structure of the face lost with time. Sculptra contains microscopic particles of poly-L-lactic acid, which is a biodegradable, synthetic material.

Sculptra works by stimulating your own collagen to build a thicker collagen matrix in the deeper layers of the skin. It is ideal when the amount of volume to be corrected is substantial, as in sunken cheeks or temporal hollows because Sculptra builds volume by stimulating the tissues to form your own collagen.

As with other injectable treatments used at MIAMI DERMATOLOGY AND LASER INSTITUTE (MDLI), local anesthesia is included in the treatment with Sculptra to keep patients comfortable. The material is injected slowly while our expert board certified dermatologists create a 3-dimensional effect to achieve a natural-looking and aesthetically pleasing result.

After 2-3 treatment sessions you can expect a to achieve up to a two year duration of the effects. Sculptra is different from hyaluronic acid fillers (Restylane and Juvederm), although they are often used in combination along with BOTOX and other neuromodulators.

Radiesse®

Radiesse® dermal filler is an FDA-approved, next-generation filler that is made of calcium-based microspheres suspended in a water-based gel. Calcium hydroxyapatite is a substance used to replace missing bone. Radiesse is injected into the skin, giving you immediate, visible improvement of folds and wrinkles, and providing long-lasting results. Offering clinically proven safety, Radiesse stimulates the body to produce new collagen. Collagen is the main structural protein found in skin. It is responsible for the supple, elastic qualities associated with youth. As the gel is absorbed by the body, it leaves the calcium hydroxyapatite in place. Your body’s natural collagen eventually fills in the spaces that are left behind. As we age, our collagen supply decreases, but Radiesse can help replenish our stores.

With Radiesse, we can achieve beautiful results and long-lasting wrinkle correction. In most cases, the effects of a Radiesse treatment may last for one year or longer.
